New Patient Registration

register_nowEveryone living in the UK is entitled to register and consult with a GP. You do not need to provide proof of ID, address, immigration status or an NHS number in order to receive care or see a GP.

New patients are required to complete a GMS1 registration form and the relevant Registration Health Questionnaire  before being accepted at the surgery.

New patients currently on medication will need to make an appointment to see a doctor for a medication review upon registering. Please bring a urine specimen with you.

Temporary Registration (For patients temporarily out of their normal practice area)

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice. You will need to register as a temporary patient.

You can be registered as a temporary patient for up to three months. This will allow you to be seen at a local practice and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use.  You cannot register as a temporary patient at a practice in the town or area where you are already registered.
