New Patient Registration

Everyone living in the UK is entitled to register and consult with a GP. You do not need to provide proof of ID, address, immigration status or an NHS number in order to receive care or see a GP.

New patients are required to complete a GMS1 registration form and the relevant Registration Health Questionnaire  before being accepted at the surgery.

New patients currently on medication will need to make an appointment to see a doctor for a medication review upon registering. Please bring a urine specimen with you.

Registering For Online Access

Under 11yrs old can be added to a parent/guardian’s access for appointments and medication.

11yrs – 16yrs no access given.

If requesting full record medical access, which is available for over 16yr old’s, a period of 30 days’ notice is required.

Below are the application forms for Systmonline, however these must be hand delivered to the surgery with your ID or the person you are authorising to have your online access.

Temporary Registration(For patients temporarily out of their normal practice area)

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice. You will need to register as a temporary patient.

You can be registered as a temporary patient for up to three months. This will allow you to be seen at a local practice and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use.  You cannot register as a temporary patient at a practice in the town or area where you are already registered.
